• Individual, Group, and Family Therapy
• Medication-Assisted Treatment (MAT)
• Holistic Wellness Programs

All services are provided by licensed healthcare professionals in accordance with applicable state and federal regulations.

3. Eligibility and Admission

To receive treatment services at 4 Seasons Recovery Center, you must:

• Be at least 18 years of age or have parental/guardian consent
• Complete our intake and assessment process
• Provide accurate and complete medical history information
• Agree to comply with all facility rules and treatment protocols
• Provide proof of insurance or arrange for payment

We reserve the right to refuse admission or discontinue services if we determine that our facility cannot adequately meet your treatment needs.

4. Patient Responsibilities

As a patient at 4 Seasons Recovery Center, you agree to:

• Actively participate in your treatment program

5. Payment and Insurance

You are responsible for understanding your insurance coverage and any out-of-pocket costs. We will verify your insurance benefits, but coverage is not guaranteed until confirmed by your insurance provider.

We are committed to protecting your privacy in accordance with HIPAA regulations and 42 CFR Part 2 (federal substance abuse confidentiality regulations). Your treatment information will not be disclosed without your written consent, except as required or permitted by law.

7. Discharge and Termination of Services

We reserve the right to discharge or terminate services for any patient who:

• Violates facility rules or policies
• Engages in violent, threatening, or disruptive behavior
• Uses substances during treatment
• Fails to comply with treatment recommendations
• Fails to make required payments
• No longer requires our level of care

In the event of discharge, we will provide appropriate referrals and transition planning when clinically appropriate.

8. Limitation of Liability

While we strive to provide the highest quality care, 4 Seasons Recovery Center cannot guarantee specific treatment outcomes. Recovery is a personal journey that requires active participation and commitment from each individual.

To the fullest extent permitted by law, 4 Seasons Recovery Center shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of our services.
